WHAT YOU SHOULD BRING TO THE EXAM

  

  • A valid passport or government-issued photo ID.
  • Your vaccination record.
  • Form I-693, Report of Medical Examination and Vaccination Record.
  • The required fee (our standard fee is $200; additional fees will be charged      for any required blood tests, vaccines and chest X-rays).
    Please note:  health insurance plans will NOT pay for this examination or related services, and we are not permitted to submit claims to insurance providers for physical examinations for immigration purposes.
  • A list of any medications that you take regularly.
  • If you have been treated for tuberculosis (TB), syphilis, or gonorrhea, a written certificate describing medications taken and results.
  • If  you have been treated for mental illness or alcohol or drug abuse, a written certificate indicating diagnosis, treatment, and prognosis

WHAT TO EXPECT

  

  • You will receive a standard physical exam. The doctor will look at your eyes, ears, nose and throat, extremities, abdomen, lymph nodes, skin, and, if necessary, external genitalia.
  • If you need vaccinations or if your records are not complete, you may choose to have blood tests that will indicate whether you are immune to specific diseases (additional fee will apply).
  • Vaccinations can also be administered by us (additional fee will apply).
  • Anyone 2 years of age or older must have a Quantiferon test. If evidence of TB is found, a chest X-ray is required (additional fee will apply).
  • Your blood will be tested for syphilis. If the test is positive, you must be treated before your I-693 form is completed.
  • Your urine will be tested for gonorrhea. If the test is positive, you must be treated before your I-693 form is completed.

Depending on the results of required tests or other parts of the exam, you might be asked to return to the clinic at a later date.

SUBMITTING THE RESULTS OF THE EXAM

  

After your exam has been finished, the doctor will complete Form I-693 and give it to you in a sealed envelope. Do not open or break the seal on this envelope. The sealed envelope must be submitted with your application to USCIS for processing.

A copy of the completed Form I-693 will also be given to you for your records. Results of your exam remain valid for one year.
